Collection Instructions

Please review all instructions and collection kit components before starting your sample collection. DO NOT discontinue taking prescription medications unless directed by your physician.

The sample must be fully dried before shipping them.
Please complete the test requisition form and health questionnaires provided.
Preparation
No special preparation needed. The test can be taken at any time.
Keep hydrated for 24 hours prior to sample collection.
Continue taking all medication unless directed by your physician.
Collection Instructions
Write the Patient Name, Date of Birth, and Collection Date on the Blood Collection (Spot) Card.
Fill all the circles on the blood spot card.
Ensure all circles are filled. Ensure the circles are filled and the blood has saturated through to the back of the card.
Air dry the card for at least 6 hours. Make sure it's completely dry.
Once the card is dry, tuck the card’s cover into its front flap to protect it and place it into the biohazard bag and seal. Then put it into the envelope.
